Large emboli that do not completely occlude vessels, particularly in patients with compromised cardiac function, may cause an acute increase in pulmonary vascular resistance. Clinical Manifestations All anemias lead to classic symptoms of decreased oxygencarrying capacity (ie, fatigue, weakness, and shortness of breath, particularly dyspnea on exertion), and iron deficiency is no exception. In comminuted fractures, special attention should be given to rotation using fluoroscopy and the well leg as a guide. Mild improvements have been reported with intravenous immunoglobulin, but the functional benefits remain doubtful [69,70]. Before discussing these interactions, a brief overview of sex- and stress-related influences on pain and analgesia will be provided. Helpful studies include chest x-ray and bone radiographs with findings suggestive of sarcoidosis or a biopsy of skin or other involved tissue showing the noncaseating granulomas characteristic of the disease.
