Pachygyria regions are also represented by thickened cortex, but wide sulci and small gyri are present. However, the patients in this study were selected for surgery based on risk, including polyp growth or suspicion for malignancy, so the relatively high risk of malignancy may have been anticipated; 20% of this group had malignancy. It usually affects the zygomatic and/or frontal bones, but other orbital bones can be affected. Many side branches end as blind pouches, but others, particularly at the hilum, communicate with each other. Contraindications to resection include multiple hepatic or distant metastases, gross vascular invasion or encasement of major vessels, malignant ascites, and poor functional status. These findings, as well as ultrastructural observations (4), support the lenticular origin of the lesion. Clinical Features In the conjunctiva, schwannoma presents as a light pink-yellow, elevated mass that generally lies in the stroma of the bulbar conjunctiva or episcleral tissues. Mucin glycoproteins account for as much as 20% of the weight of black pigment stones. A leading possibility is that it represents a tumor of neural origin, perhaps a malignant variant of a paraganglioma (chemodectoma) or granula cell tumor (1,2). Progressive hepatic injury can occur during the first months of life as a result of biliary obstruction caused by poor bile flow, sludge, protein plugs, and stones composed of fatty acids and calcium. The same data on frequency of new stroke cases are reported for the United States population, and it was mentioned that 80% of all these cases are ischaemic stroke as opposed to haemorrhagic stroke (which accounts for the remaining 20%). The crystals can be detected by polarizing light microscopy in a sample of bile previously rendered crystal-free ("isotropic").
